Please give a full medical diagnosis of the disease.
An approval for the visa for the operation by American doctors, including the estimate for the operation with breakdown of the costs.
State your full name and address.
Tell if you are willing to travel to Moscow for a second opinion. My wife's parents are among the top doctors in Russia and will check and confirm the diagnosis, also they will advise if the operation can be performed only in the USA or better in other countries/places.
