There's nothing on this form that says it should be notarized, at least not the version I downloaded from the government website. It was updated in January of 06, so it should be up to date. But my wife says I needed it notarized. I got it notarized, though the notary didn't know where ot put his mark -- he photocopied something onto the back page, then stamped and signed and all that.

Another thing, are you supposed to send copies of your tax return and all your various W2s and 1099s, or are you supposed to send transcripts, obtained from the IRS, of your most recent tax return, and perhaps your previous two? It asks for only copies, but in various searches online I found people saying you need transcripts -- which I wouldn't even be able to get, I'm sure, as I filed last week, after receiving an extension to Oct. 15, because I was living overseas.

This process is crazy. There are so many potholes you can land in. I'm hoping I got everything right, b/c I gave it all to DHL and they carried it away at a cost of $100.
