Married name on Ukr. passport

I would like to have my wifes married name put on her Ukr. international passport. Does anyone know the process for this? I understand they affix some sort of sticker? I've tried to call all the consulates here but can never speak to a live person.

All I know is of a particular situation where the name on her Ukrainian passport did not match her other documents (driver's license, SSA card) showing her married name. At JFK airport they would not issue her a boarding pass until her husband faxed a copy of the marriage certificate. After this incident she always carries a photocopy of her marriage certificate whenever she returns to Ukraine.
