Adjustment of Status questions

I am in the process of completing our Adjustment of Status paperwork.  I went to my local USCIS office on an INFOPASS to try to get my questions answered.  The gentleman knew less than I did about AOS for K1 visas.  My first question relates to the vaccination records.  Is submitting the form DS-3025 that my wife was given acceptable? Or do we need to have a doctor transfer this information to an I-693 appendix A?  And should it be sealed? 

My local office actually told us we needed a new medical exam and a new complete, sealed 693 or my wife would never receive a green card.  When I showed him in the 485 instructions that K1 medical exams within the past year only need the vaccination supplement, he said it was not correct.

My second question relates to the I-864 Affidavit of Support.  On the USCIS website I read that I only need to submit my most recent year's tax return and proof of income.  I was told by my local office that I must submit 3 years of returns.

Any help with both of these questions from those that have recently been through this process would be greatly appreciated.

My information and advice goes back 4 years.

But here goes.

I live in Florida and I mailed my packet to the National Address (Chicago Lockbox).

I mailed in my wife's vaccination record (form DS-3025) and her chest X-Ray (from Moscow) and on my cover letter I reminded them that her medical exam in Moscow was still good (4 month's old).

I never had any troubles (got GC), BUT...My wife's friend from Omsk who came behind her one month later, had all kinds of troubles (and still is). She married a guy and lives in Las Vegas. Their file is in California. They filed the paperwork the same as us, but it didn't work for her. She had to have another medical exam, etc.

By the way, she had to find a "special" doctor. See below:

x633 we also are currently in the process. Interview comming up on the 10th.

Medical exam is good for 1 year. So if your wifes exam for a k1 was done less than a year ago , you do not need another.

You should have a copy of the vaccination record that they gave you when she had the exam. If her vacinations were not complete, (most likely needs chicken pox possible) she just needs to go to  civil surgeon, have the vaccination done, and a new record updated. Take your old one with you.

Thanks for your reply.  I did use Visa Journey for many of the forms.  I just could not find a clear answer on which vaccination form should be submitted. Good luck with your interview and the process.  Thanks also for the cover letter.  It is very helpful.

The USCIS website is correct.

You must submit the last income tax filed.  You only need to provide the numbers (adjusted gross income) for years two and three.

We had an immigration attorney do ours, and that is what he said to provide.

It is required that you show employment, or other sources of income (retirement, stocks, investments, etc.).  In my case, my employer wrote a letter stating that I was employed at x number of dollars. That went in with the submission.

I was told by my local office that I must submit 3 years of returns. 

We found on several occasions that our local office clerks didn't know the information on
their own official website. When we walked in back in 2003 to demand Advance Parole,
we produced copies of their stated policy regarding timeframe. They issued the document
within an hour.

Re: employment letter - it doesn't hurt to have them mention your longevity as well.
