Child traveling abroad / worries with ex husband

I'm coming back on board.
I get some worries with the ex of my girlfriend.

I would like to know if, in Ukraine, the letter done by the father of a child to allow him to go abroad is a free formula letter or an official form. And what must be in the letter ? date of travel, destination ?

This is in connection with the child residing abroad as a permanent resident?

Other than identification of the parents and child, the letter needs to state that the non custodial parent is giving his/her permission for the child to reside in whatever country as a permanent resident.

The letter must be proved up as a legal document. I believe that the US consulate has the requirements for prove up of permission still posted on its website.

This is just for 2 weeks of holidays in Turkey. Next year will be for permanent resident if everything is fine and destination will be Canada
I just want to know what is needed for her child (7 years old) to come with us, to be sure there will be no issue at the custom when we will be leaving Kiev.
Her ex was ok first, negative after but now he just want to have a letter ready to sign ...

I am not sure of her source of information and I prefer to double check with people who had the same experience .

General temporary consent letters need to identify the child, destination, and duration of visit, signed, notarized, and apostiled
