I am doing the paperwork and I have the form dated 09/11/00 Y
This is the same as the one on VJ sample forms, EXCEPT theirs says submit all four pages, mine says "submit both copies of this form"

So the same revision date, one has 4 pages (last 3 are identical except for the number at the bottom).

Crap. Anyone run into this?

No sure if I follow you, but my G-325 has four parts, all of which must be submitted because, at least in principle, they go to different people.  The on-line fill in forms make this painless, so it wasn't an issue with me.

Somehow I got a copy online as catz linked me, that has 2 parts. The revision date is the same as the 4 part one I just found, but on mine, it says both pages, not all four copies.

They gave you the link to the G-325. It is a 2 page document.

What you need is a G-325A. it is here: http://uscis.gov/graphics/formsfee/forms/files/g-325a.pdf

There are also G-325B, and G-325C which are 1 page documents

Oosik,

Here's what I recall. The first set of 325's , 4 copies are required. Once she's here and you aply for AOS only 2 copies are required.
