I'm not sure I got the question correctly, but each Ukrainian passport does have a unique number.

I have the passport number and personal number. But I am unsure if the authority number in the passport is the passport book number. Not all countries
use a passport book numbers. I'm filling out the DS-160 form.

No, Authority number in the passport - it's not passport number. I have a blue-colored passport (there are 3 versions of Ukrainian passports), and in my version Passport No. is written at the top right corner of the first page, and perforated in each page of the passport. That is passport number. Authority no. is written only in the middle of the first page and nowhere else. It's a code of the organisation which issued the passport. But I've never filled out the DS-160, only DS-156, and in that form the passport number (which is perforated in each page) goes in the first field.

Update:

Ukraine does not use a passport book number.

My wife's mother was granted a visa this past week. We filled out and singed the online DS-160 form for her at the end of April. We waited a few days to buy a pin code and make a appointment. The earliest date we could get was June 8. She brought proof of bank accounts and properties. A letter from her boss and copies of family members passports. They did not ask for any of it, but asked many questions. The questions asked were about her, her husband and about us. The interview was about 15 to 20 minutes. She said the guy was nice, but strict.

Do you mean you filled out and signed (electronically) the DS-160 from your computer in USA, while your MIL was in Ukraine?

Did  you consider the risk?

They can trace the ISP of computer used, and when you click those buttoms there is an oath that you are the visa applicant.
A beautiful woman is pleasant to look at, but it is easier to live with a pleasant acting one.

I filled it out as the third party and gave my info. I do not know if you had used the online form, but it is buggy. It would not save travel dates and crashed many times. It also would not save the photo we uploaded. We felt it was more important the form was filled out correctly. We had talk with others who had done it this way with success.

Yes, I understand that it is allowed to get help with the form, and then the third party tells their info.

But as I recall, it very clearly states that only the applicant can click the final button to submit since they are giving the oath, etc.

I know I helped the one of my gals with the Student Visa application as she is quite a computer novice, but when I got to the part about clicking the final button, I said:  NO, No, I am not going that route.

And yes, I know about the difficulty of dealing with  the form.  It says you have 20 minutes before it shuts down . . . but it actually only gives about 4-5 minutes before it shuts down!!  So I did a 'save' about every two minutes.
