Do tax forms need to be notarized?

We have our K3 interview on Monday March 28th. Just going over everything to make sure everything is in order. Does appear so. However I noticed on 1 form & ONLY that one form it states the income taxes statements need to be NOTARIZED. However it says for the COPIES. Do I need to have the income tax statements notarized or not? Also will I need to submit 2 photos of ME as well? I think not however I was told to take some by my atty who personally I think is clueless, at least more so than me. :D Seriously all in all it appears we're almost there.
