Question about I-325 Bio form for AOS

Hi. I've been a long time lurker, and have enjoyed reading the advice given on this forum. My fiancee arrived from Volgograd on 10-28-08, and we were married on Dec. 27. Now, as I begin the paperwork for AOS and after reading the VisaJourney website, I've got a few questions:

   1. Do I need to submit a set of the I-325A biographical forms for both my wife AND myself, or just for my wife?

   2. On my wife's I-325, at the top where it asks for Family Name, should she use her new last name (mine), or would she use her maiden name?

   3. Where it says "...This form is submitted in connection with an application for..." and the choices are: Naturalization, Other, and Status as Permanent Resident, would she select Other: I-485 Petition, or would she choose Status as Permanent Resident?

  4. Does she sign the form using her married name or her maiden name?

  5. Lastly, in the dark box at the very bottom, would she put her maiden name or her married name?


Thanks for any help that can be offered. I've tried searching this forum and others for the answers to the above questions.

Yep. Already tried it, and I've printed out their example. However, it is for someone not yet married, and thus my questions.

I believe this goes like this - AOS case:

1. G-325A for both of you
2. Use her new (your) last name.
3. Status as Permanent Resident
4. She signs using her married name and in Russian writes her full name with your last name

5. Again in the dark box she should put her new married name - that is yours.

Anastassia is right.  Your wife should remember that since now she should be using her new last name (yours) on all documents and forms unless it specifically asks for her maiden name.
